This part-time position supervised by the Cook/Manager is responsible for assisting the Cook/Manager in all
aspects of preparation of nutritious and attractive meals, and good sanitation practices according to procedures and
guidelines as set by the Nutrition Department.

Job Title:            Case Manager

Department:      Case Management Agency

Starting Wage:   $ 21 - 22.00

Locations:           MorganLogan and Kit Carson Counties

General Job description:

This full-time position is responsible for performing a variety of highly responsible, confidential, and complex
duties including providing professional and technical assessment, case management and direct service and assistance
to clients. Clients may include the elderly, blind, disabled, brain injured, mentally ill, developmentally disabled, and
those in hospice, pediatric and home health. Travel is required, must be able to manage a caseload of 80+, and be
detailed oriented and a team player.

Job Title:            Part time Driver

Department:      Transportation

Starting Wage:   $ 16.00

Locations:           Morgan and Yuma County

General Job description:

​

The Part Time Driver is responsible for safe transportation of clients in the Northeastern Counties as well as trips out of the area. Drivers must work as a team with dispatch centers, other drivers, supervisors and the central office to coordinate passengers. It is necessary to be self-motivated and be able to work effectively little direct supervision. This is a hands-on position, and the ideal candidate will be one who has compassion and the ability to work with the elderly and potentially disabled individuals.

Department:      CMA

Starting Wage:   $ 18.00/ hour Depending on Experience

Locations:           Morgan County

General Job description:

​​This full-time position provides administrative and operational support to Case Managers and other
team members within the Case Management department. Accepts referrals from internal sources,
other county or state human service agencies, local medical facilities, nursing homes and assigns case
to Case Manager. Performs follow-up activities related to ongoing case management. Tracks
information requests and manages member information in multiple computerized databases.
